IgG4-related disease is a chronic, relapsing inflammatory condition that can affect almost any organ in the body. It is characterized by the presence of IgG4-positive plasma cells in affected tissues, often leading to fibrotic changes and the formation of tumor-like masses. The exact cause of IgG4-related disease is unknown, but it is thought to be an immune-mediated process.

The clinical presentation of IgG4-related disease is highly variable, depending on the organs involved. Common manifestations include dacryoadenitis (inflammation of the lacrimal glands), sialadenitis (inflammation of the salivary glands), pancreatitis, cholangitis, retroperitoneal fibrosis, and interstitial nephritis. Patients may experience a wide range of symptoms, such as pain, swelling, organ dysfunction, and constitutional symptoms like fatigue and weight loss.

Diagnosis of IgG4-related disease typically involves a combination of clinical evaluation, imaging studies, serological tests (elevated serum IgG4 levels), and histopathological examination of biopsy specimens. The biopsy should demonstrate characteristic IgG4-positive plasma cell infiltration, fibrosis, and obliterative phlebitis.

Treatment for IgG4-related disease usually involves immunosuppressive therapy. Glucocorticoids are often the first-line treatment, with significant improvement seen in many patients. For those who do not respond to or tolerate glucocorticoids, other immunosuppressive agents such as azathioprine, mycophenolate mofetil, or rituximab may be used. Management is often long-term due to the chronic and relapsing nature of the disease. Monitoring for disease activity and potential complications is crucial.
