HLA-related refers to aspects connected with the human leukocyte antigen (HLA) system, a cluster of highly polymorphic genes on chromosome 6 that encode major histocompatibility complex (MHC) cell-surface proteins. These proteins are central to immune recognition, presenting peptide fragments to T cells and guiding the adaptive immune response.

HLA genes are divided into class I (HLA-A, -B, -C) and class II (HLA-DR, -DQ, -DP). Class I molecules present endogenous peptides to CD8+ T cells, while class II molecules present exogenous peptides to CD4+ T cells. The extreme diversity of HLA alleles across individuals and populations underpins differences in antigen presentation and immune reactivity. The region also contains many regulatory elements and exhibits strong linkage disequilibrium, creating common haplotypes with population-specific patterns.

HLA typing is routinely used in transplantation to assess compatibility between donor and recipient. Matching major loci such as HLA-A, -B, -C, and HLA-DRB1 (and sometimes HLA-DQB1) reduces the risk of graft rejection and graft-versus-host disease and improves transplant outcomes. In hematopoietic stem cell transplantation, higher-resolution typing is particularly important.

Numerous HLA associations have been described with diseases and drug responses. HLA-B27 is linked to spondyloarthropathies; HLA-B15:02 and carbamazepine can trigger Stevens-Johnson syndrome in specific populations; HLA-B57:01 is associated with abacavir hypersensitivity; HLA-DQ2 and DQ8 are strongly linked to celiac disease; certain DRB1 alleles carry the rheumatoid arthritis “shared epitope.” HLA variation also influences infection susceptibility and progression, as well as responses to vaccines and medicines.

Research into HLA contributes to immunology, transplant medicine, autoimmune disease understanding, pharmacogenomics, and population genetics, while raising considerations about genetic privacy and equitable access to testing.
