HAZOP analyses, or Hazard and Operability studies, are a systematic and structured risk assessment technique used to identify potential hazards and operability problems within a process or system. It is a qualitative approach that involves a multidisciplinary team examining a process design or operation. The team typically comprises individuals with expertise in the process, safety, operations, and engineering.

The core of a HAZOP analysis involves breaking down the process into discrete sections or "nodes." For each node, a team of specialists systematically applies a set of guidewords, such as "No," "More," "Less," "As Well As," "Part Of," "Reverse," and "Other Than," in conjunction with process parameters like "Flow," "Pressure," "Temperature," and "Level." This combination helps to provoke brainstorming and identify deviations from the intended design or operation.

For each identified deviation, the team investigates the potential causes and consequences. Consequences can range from minor operational upsets to catastrophic accidents, including fires, explosions, and toxic releases. The team then proposes safeguards or recommendations to mitigate or eliminate the identified risks. These recommendations are often prioritized based on the severity of the potential consequences and the likelihood of occurrence.

HAZOP analyses are widely used in industries such as chemical processing, oil and gas, pharmaceuticals, and nuclear power. They are typically conducted during the design phase of a project to ensure safety is built into the process from the outset, but they can also be applied to existing facilities to identify and address latent hazards. The documentation of the HAZOP process, including identified hazards, causes, consequences, and recommendations, forms a crucial part of the safety case for a facility.
