Estolate

Estolate is a chemical compound often encountered as a salt of erythromycin. Specifically, it is the lauryl sulfate salt of erythromycin. This form is created to improve the oral absorption and stability of erythromycin, an antibiotic. Erythromycin itself is effective against a wide range of bacterial infections, particularly gram-positive bacteria.

The estolate form of erythromycin is designed to be more resistant to degradation by stomach acid, allowing a greater amount of the active drug to reach the intestines for absorption into the bloodstream. This enhanced absorption can lead to higher and more consistent blood levels of the antibiotic, potentially improving its efficacy. It is commonly prescribed for conditions such as respiratory tract infections, skin infections, and certain sexually transmitted infections.

However, estolate is also associated with a specific type of adverse reaction. It has been linked to a higher incidence of hepatotoxicity, or liver damage, compared to other erythromycin salts. This liver injury is typically cholestatic in nature, meaning it affects the flow of bile from the liver. Symptoms can include jaundice, abdominal pain, and elevated liver enzymes. Due to this risk, estolate is often avoided in patients with pre-existing liver conditions or when other erythromycin formulations are suitable. The decision to use estolate is usually made by a healthcare professional weighing the benefits against the potential risks.
