Efterforløb

Efterforløb is a Danish term describing the period following an initial intervention, treatment, or program during which monitoring, support, and further actions are organized to secure outcomes and continuity of care. The word combines efter (after) and forløb (course or progression).

In healthcare, aftercare encompasses follow-up activities after hospital discharge or after procedures such as surgery, chemotherapy, or radiotherapy. The aim is to detect complications early, manage symptoms, prevent relapse, and support adherence to prescribed regimens. Outside medicine, the term is used in social services, education, and rehabilitation to describe structured post-treatment or post-program support, including assessments, care coordination, and referrals.

A typical aftercare arrangement includes an initial assessment of needs, a written follow-up plan, scheduled appointments or contacts, monitoring of progress, empowerment and education for patients, and referrals to allied professionals or community resources. The exact content and duration vary by condition, risk level, and local guidelines.

Benefits of aftercare include improved health outcomes, reduced readmissions, better adherence, and smoother transition between care settings. Challenges can include limited resources, coordinating among multiple providers, patient engagement, and data sharing.
