Dosisregime

Dosisregime, or dosing regimen, is a structured plan that specifies how a drug should be given. It covers the dose per administration, the interval between doses, the route of administration, and the total duration of therapy. The goal is to achieve therapeutic drug concentrations while minimizing adverse effects and, when relevant, the development of resistance.

A regimen may include a loading dose to rapidly achieve target levels, followed by maintenance dosing. It may specify titration to reach a desired effect, and may include as-needed dosing for symptom relief. Regimens are adapted to individual factors such as age, weight, renal or hepatic function, pregnancy, and potential drug interactions. In some cases, continuous infusion or stepwise escalation is used.

Development and selection of a dosing regimen rely on pharmacokinetic and pharmacodynamic principles, clinical guidelines, and patient-specific data. Standards may be adjusted for special populations (pediatrics, geriatrics) and settings (outpatient vs hospital). Therapeutic drug monitoring is used for certain drugs to ensure concentrations remain within a therapeutic range.

Adherence and monitoring are central to regimen success. Missing doses, taking incorrect amounts, or accumulating doses can reduce efficacy or cause toxicity. Regular review may be needed to adjust the regimen in response to changing clinical status, lab results, or adverse effects.
