Dermomyotom

Dermomyotomy is a surgical procedure that combines an incision through the skin with division of underlying muscle tissue. The term derives from dermis, myotomy, and denotes a technique intended to relieve restriction by addressing both cutaneous and muscular components in a single operation.

Indications for dermomyotomy are historical and include release of contractures where the skin and muscle contribute to limitation of movement, or to provide access to deeper structures during reconstructive operations. In contemporary practice, explicit use of the term dermomyotomy is uncommon, and surgeons typically describe the separate steps (skin incision and muscle division) or prefer other release procedures such as fasciotomy or targeted muscle releases.

Technique involves performing the operation under sterile conditions with careful planning of the skin incision to expose the underlying muscle. The involved muscle is then divided along its fibers to release tension. Hemostasis is achieved, the wound is closed in layers, and postoperative care includes immobilization and rehabilitation tailored to the condition being treated.

Risks associated with dermomyotomy include infection, hematoma, scarring, sensory or motor nerve injury, muscle weakness, and recurrence of contracture. Functional outcomes depend on the extent of release, the underlying pathology, and adherence to rehabilitation protocols.
