Colonsmanometrie

Colonsmanometrie, also called colonic manometry, is a diagnostic procedure used to assess the neuromuscular function of the colon. It involves inserting a flexible, pressure-sensing catheter into the colon through the rectum and advancing it to different segments. The catheter records intraluminal pressures over a period of time under resting conditions and in response to provocative stimuli, allowing detailed mapping of colonic motility.

Indications for colonsmanometrie include evaluation of severe or refractory constipation, suspected colonic inertia, and other unexplained motility disorders when non-invasive tests do not provide clear answers. It may also be used in research settings or to study motility patterns in patients with fecal incontinence or after colorectal surgery to understand residual function.

Procedure and interpretation: The test is typically performed in a motility laboratory. After bowel preparation, the patient undergoes catheter placement with careful advancement to the colon. Recordings document baseline activity and responses to stimuli such as meals or pharmacologic challenges. The data are analyzed for patterns including slow-transit features, high-amplitude propagating contractions, and other characteristic motility signatures. Normal ranges vary, and results are interpreted by specialists in gastrointestinal motility in the context of symptoms and other diagnostic information.

Risks and limitations: Colonsmanometrie is an invasive procedure and carries risks such as discomfort, bleeding, or rarely colonic perforation, especially in less experienced hands. Availability is limited to specialized centers, and results can be variable, requiring interpretation by trained clinicians. The procedure is typically considered when the potential diagnostic benefit outweighs these risks.
