Alaryhmätutkimukset, often translated as subgroup analyses or secondary analyses, are statistical investigations conducted within a clinical trial or observational study to explore outcomes in specific subsets of the overall study population. These subsets are typically defined by baseline characteristics such as age, sex, disease severity, or treatment history. The primary purpose of subgroup analyses is to determine if the observed treatment effect or association is consistent across different patient groups or if it varies significantly.

While these analyses can offer valuable insights into the heterogeneity of treatment effects and potentially identify subgroups that might benefit more or less from an intervention, they are often viewed with caution by the scientific community. This is because subgroup analyses are usually exploratory in nature and are not powered to detect statistically significant differences within these smaller groups. Performing numerous subgroup analyses increases the risk of finding statistically significant results purely by chance, leading to false-positive conclusions.

The interpretation of subgroup analyses should therefore be done with great care. They are best used to generate hypotheses for future research rather than to draw definitive conclusions about treatment efficacy in specific populations. If a subgroup analysis shows a notable difference, it should ideally be confirmed by a prospective, pre-specified subgroup analysis in a separate trial or a larger, well-powered study. This approach helps to mitigate the risk of spurious findings and increases the reliability of the results.
