Transplacental refers to the passage of substances or cells from the maternal circulation to the developing fetus across the placenta. The placenta serves as an interface between maternal blood and fetal circulation, with a barrier formed mainly by trophoblast layers and fetal endothelium that regulates exchange and helps protect the fetus from certain maternal components.

Transplacental transfer occurs through multiple mechanisms. Some transfers are beneficial, most notably the active transport of maternal IgG antibodies across the syncytiotrophoblast via Fc receptors, providing the fetus with passive immune protection. Nutrients and gases cross through specific transporters and diffusion. Other agents can cross and may pose risks, including certain medications, toxins, and infectious agents such as human immunodeficiency virus (HIV), cytomegalovirus, toxoplasma gondii, rubella, and syphilis, as well as teratogenic drugs like isotretinoin or thalidomide. The likelihood and impact of transfer depend on gestational timing, the substance’s properties, and overall placental function.

Clinical relevance includes informing vaccination timing, assessing fetal risk, and managing maternal infections and drug exposures. Understanding transplacental transfer helps in evaluating fetal exposure to nutrients, antibodies, medications, and pathogens, as well as in research contexts where placental transport mechanisms are studied to predict fetal drug exposure and outcomes.
