The smittekjede typically involves several links. The first is the infectious agent itself, the pathogen causing the illness. This agent must have a reservoir, which is where it lives and multiplies, such as a person, animal, or even the environment. From the reservoir, the agent must find a portal of exit, a way to leave the reservoir, like through coughing, sneezing, or bodily fluids.

Transmission then occurs through a mode of transmission, which can be direct, such as person-to-person contact, or indirect, via contaminated objects, water, or air. Following transmission, the agent enters a susceptible host through a portal of entry, such as the respiratory tract, skin breaks, or mucous membranes. If the host's immune system is unable to fight off the infection, they become infected, and the smittekjede can continue.
