Ryanodine Receptor (RyR) is a protein found in the sarcoplasmic reticulum (SR) of muscle cells, responsible for the release of calcium ions (Ca2+) during muscle contraction. It is a member of the inositol trisphosphate receptor (IP3R) family of calcium channels. The RyR protein is a tetramer, consisting of four identical subunits, each with six transmembrane domains and a large intracellular domain.

The RyR protein functions as a calcium release channel, allowing the rapid and controlled release of calcium ions from the SR into the cytoplasm. This calcium influx triggers muscle contraction by activating troponin, a protein complex that regulates the interaction between actin and myosin filaments. The RyR protein is essential for muscle function and is involved in various physiological processes, including heart contraction, smooth muscle relaxation, and neurotransmitter release.

Mutations in the RyR gene can lead to inherited muscle disorders, such as malignant hyperthermia (MH) and central core disease (CCD). MH is a rare but potentially life-threatening condition characterized by a rapid increase in body temperature and muscle rigidity in response to certain anesthetics or muscle relaxants. CCD is a progressive muscle disorder that affects the heart, skeletal muscles, and smooth muscles, leading to weakness, fatigue, and arrhythmias.

The RyR protein is a target for various pharmacological agents, including ryanodine, a plant alkaloid that binds to the RyR protein and inhibits calcium release. Ryanodine is used in the diagnosis of MH and CCD, as well as in the treatment of other muscle disorders. Other RyR modulators, such as caffeine and felodipine, are also used in the treatment of muscle disorders and have been shown to have potential therapeutic applications in other diseases, such as heart failure and neurodegenerative disorders.
