Pupillary is an anatomical and medical term relating to the pupil, the circular opening in the center of the iris through which light enters the eye. The pupil’s size and responsiveness are essential for regulating retinal illumination and supporting clear vision across different lighting conditions.

The pupil is controlled by the iris, which contains two smooth muscle groups that regulate diameter. Dilation (mydriasis) is produced by the dilator pupillae, a radial muscle that expands the pupil under sympathetic stimulation. Constriction (miosis) is produced by the sphincter pupillae, a circular muscle that narrows the pupil under parasympathetic stimulation from the oculomotor nerve. The autonomic balance between these pathways adjusts pupil diameter as needed.

Pupillary reflexes and testing are important in clinical practice. The pupillary light reflex involves a direct response in the illuminated eye and a consensual response in the opposite eye, assessing the afferent pathway (optic nerve) and the efferent pathway (oculomotor nerve) to the iris. The near reflex or near response includes pupil constriction when shifting focus to a near object, along with accommodation and convergence. Abnormal pupillary responses can indicate neurological or ophthalmic problems.

Clinical relevance extends to conditions that produce anisocoria, or unequal pupil sizes, which can arise from benign variation or from lesions affecting the sympathetic or parasympathetic systems, such as Horner’s syndrome or third nerve palsy. Pharmacologic agents, trauma, and disease can also alter pupil size. Normal variation exists with environmental lighting, age, and individual physiology, with typical adult pupils ranging roughly from 2–4 mm in bright light to 4–8 mm in darkness.
