Nielemäprosessi refers to the complex physiological action of swallowing, which allows food, liquid, or saliva to pass from the mouth to the stomach. This process is crucial for nutrition and hydration. It is a coordinated sequence of voluntary and involuntary muscle contractions that involves several cranial nerves and multiple anatomical structures. The swallowing process is typically divided into three phases: the oral phase, the pharyngeal phase, and the esophageal phase.

The oral phase begins with the manipulation of food or liquid in the mouth, forming a bolus. This phase is under voluntary control. The tongue propels the bolus towards the pharynx.

The pharyngeal phase is an involuntary reflex. As the bolus enters the pharynx, the soft palate elevates to prevent nasal regurgitation, the epiglottis covers the larynx to protect the airway, and the vocal cords close. Muscles in the pharynx contract to propel the bolus downwards.

The esophageal phase is also involuntary. Once the bolus passes the pharyngeal constrictors, it enters the esophagus. Peristalsis, a wave-like muscular contraction, moves the bolus down the esophagus towards the stomach. The lower esophageal sphincter relaxes to allow the bolus to enter the stomach and then closes to prevent reflux.

Dysphagia, or difficulty swallowing, can occur if any part of this intricate process is impaired. It can result from neurological conditions, structural abnormalities, or aging. Swallowing disorders can lead to malnutrition, dehydration, aspiration pneumonia, and a reduced quality of life. Assessment and management of dysphagia often involve speech-language pathologists and other healthcare professionals.
