Megakaryopoiesis, or megakaryopoiesis, is the development of megakaryocytes from hematopoietic stem cells in the bone marrow and the subsequent production of platelets. The process involves lineage commitment to the megakaryocyte-erythroid pathway, maturation of megakaryocytes, endomitosis to increase DNA content without cell division, and fragmentation of the cytoplasm to release platelets into the bloodstream.

Thrombopoietin (TPO) is the principal regulator of megakaryopoiesis. It signals through the c-MPL receptor and activates JAK-STAT, MAPK, and PI3K pathways to promote progenitor proliferation, differentiation into megakaryocytes, and terminal maturation. Other cytokines, including interleukins such as IL-3, IL-6, and IL-9, support early progenitor expansion and later maturation, but TPO remains the key driver of platelet production.

Anatomically, megakaryocytes reside predominantly in the bone marrow, where their cytoplasm extends into sinusoidal vessels. Through proplatelet processes, platelets are released into the circulation, where they function in hemostasis and wound repair. Although the bone marrow is the primary site of platelet production, there is evidence that platelets can also be produced in small amounts in the lungs under certain conditions.

Clinical relevance centers on the regulation of megakaryopoiesis and platelet counts. Abnormal megakaryopoiesis can lead to thrombocytopenia or thrombocytosis. Disorders such as essential thrombocythemia and other myeloproliferative neoplasms involve dysregulated megakaryocyte proliferation. Diagnostic assessment includes complete blood counts and bone marrow examination, with megakaryocytic markers like CD41 and CD61 aiding lineage identification.
