Kontralateralt, or contralateral, describes something that is located on, or affects, the opposite side of the body or of a structure. The term is used in anatomy, physiology, and medicine to express spatial relationships between the left and right sides or between paired organs, such as the contralateral kidney being on the opposite side from another organ.

In neuroanatomy, contralateral organization is common because many neural pathways cross the midline. For example, motor signals originating in the left cerebral cortex largely control the muscles on the right side of the body, due to the decussation of the pyramids in the brainstem. Similarly, many somatosensory pathways cross to the opposite side so that sensory information from one side of the body is processed in the opposite hemisphere. Some pathways remain uncrossed or cross at different levels, which can complicate the pattern of deficits following injury.

Clinically, contralateral deficits are often observed after brain or spinal injury. A stroke in the left hemisphere may produce contralateral (right-sided) weakness or sensory loss, and right-hemisphere damage can lead to left-sided neglect in some patients. The term also appears in descriptions of paired organs, such as contralateral kidneys or eyes, indicating a relation to the opposite side.

Kontralateralt is used across Scandinavian languages with the same general meaning, emphasizing the opposition of sides in anatomical and clinical contexts.
