The term frontooccipital refers to structures or relationships that involve both the frontal and occipital lobes of the brain. It is most commonly used in two contexts.

First, the frontooccipital fasciculus is a white‑matter fiber bundle that runs from the medial frontal cortex to the occipital pole. It is part of the superior longitudinal fasciculus complex and is thought to support visual–executive integration, spatial awareness, and the coordination of eye movements. Diffusion tensor imaging studies have shown that the tract has a dorsolateral trajectory, passing through the posterior limb of the internal capsule and the anterior portion of the splenium of the corpus callosum. Disruption of this tract can lead to deficits in visual attention and abstraction, and it has been implicated in the neuroanatomical substrate of certain forms of dyslexia and in the visual processing disturbances seen in schizophrenia.

Second, the phrase frontooccipital can describe the broad border of the organization of the cortex between the frontal and occipital lobes. In anatomy, the term is sometimes applied to the foveal region of the primary visual cortex (Brodmann area 17), which lies near the calcarine sulcus, and to the neighboring portions of the frontal eye fields that lie just anterior to the temporal lobe. The contact between the frontal eye fields and the occipital visual stream is essential for saccadic eye movements and for the planning of visually guided actions.

In clinical neurology, lesions that involve the frontooccipital region may produce a combination of executive dysfunction, visual field deficits, and impaired ocular control. Neuroimaging plays a critical role in identifying such lesions and monitoring the integrity of the frontooccipital fasciculus in conditions ranging from traumatic brain injury to neurodegenerative disease.
