Bronchileft refers to the left main (primary) bronchus, one of the two principal airways that branch from the trachea to supply the lungs. It conducts air from the trachea into the left lung and serves as a conduit for subsequent divisions of the bronchial tree.

Anatomy and course: The left main bronchus originates at the carina, near the level of the tracheal bifurcation, and descends obliquely to the left. It is typically longer and narrower than the right main bronchus. It passes behind the aortic arch and in front of the esophagus as it approaches the left lung, entering the lung at the hilum.

Branches and distribution: The left main bronchus usually divides into the left upper lobe bronchus and the left lower lobe bronchus. These lobar (secondary) bronchi then give rise to the segmental (tertiary) bronchi that supply the bronchopulmonary segments of the left lung. The left lung has two lobes (upper and lower), and the left main bronchus provides the majority of the airways to both lobes.

Clinical significance: The left main bronchus can be affected by obstruction from tumors, foreign bodies, or inflammatory processes, leading to collapse or infection of the left lung or its lobes. The anatomy of the left bronchus, being longer and more angled than the right, influences approaches to endotracheal intubation and selective bronchial intubation used in certain procedures. Imaging and bronchoscopy commonly assess the left main bronchus to evaluate airway patency and pathology.

Variations: Minor anatomical variations in branching patterns or course exist among individuals. Knowledge of these variations aids in radiologic interpretation, bronchoscopy, and thoracic surgical planning.
