Bilirubinmetabolizing refers to the complex biological process by which the body breaks down and eliminates bilirubin, a yellowish pigment produced during the normal breakdown of red blood cells. This metabolic pathway is crucial for preventing the accumulation of bilirubin, which can lead to jaundice and other health issues.

The process begins with the breakdown of hemoglobin, the protein in red blood cells that carries oxygen. When red blood cells reach the end of their lifespan, typically around 120 days, they are cleared from circulation by macrophages in the spleen, liver, and bone marrow. Within these macrophages, hemoglobin is catabolized into heme and globin. The heme molecule is then further processed to release iron, which is recycled, and a porphyrin ring. This porphyrin ring is converted into biliverdin, a green pigment.

Biliverdin is subsequently reduced to bilirubin by the enzyme biliverdin reductase. At this stage, bilirubin is unconjugated, also known as indirect bilirubin. Unconjugated bilirubin is lipid-soluble and binds tightly to albumin, a protein in the blood, for transport to the liver.

In the liver, unconjugated bilirubin is taken up by hepatocytes. There, it undergoes conjugation, a process that makes it water-soluble. This is primarily achieved by attaching glucuronic acid to bilirubin, forming bilirubin diglucuronide, also known as conjugated or direct bilirubin. This conjugation is facilitated by the enzyme UDP-glucuronosyltransferase.

The conjugated bilirubin is then secreted from the hepatocytes into the bile and transported to the gallbladder for storage or directly into the small intestine. In the intestine, gut bacteria further metabolize conjugated bilirubin into urobilinogen. A portion of urobilinogen is reabsorbed into the bloodstream and excreted by the kidneys as urobilin, which gives urine its characteristic yellow color. The remaining urobilinogen is further metabolized into stercobilin, which is excreted in the feces, giving stool its brown color. Disruptions at any stage of this pathway can lead to elevated bilirubin levels and clinical manifestations of jaundice.
