The subscapularis is a large, fan-shaped muscle on the anterior surface of the scapula. It originates from the subscapular fossa and inserts onto the lesser tubercle of the humerus, with some fibers contributing to the shoulder joint capsule. Its tendon blends with the anterior part of the joint capsule and with surrounding rotator cuff tendons.

It is innervated by the upper and lower subscapular nerves (C5–C7) from the posterior cord of the brachial plexus. Functionally, it medially rotates the humerus and assists in adduction, helping to stabilize the glenohumeral joint during shoulder movements.

Blood supply comes primarily from the subscapular artery, with contributions from adjacent branches. The muscle works in concert with the other rotator cuff muscles to maintain humeral head stability and facilitate overhead and throwing motions.

Clinically, tears or tendinopathy of the subscapularis can cause weakness in internal rotation and anterior shoulder pain. Diagnosis may involve strength testing and imaging, and treatment ranges from physical therapy to surgical repair in some cases.
