3-hydroxybutyrate, also known as beta-hydroxybutyrate or BHB, is a ketone body produced in the liver from the breakdown of fatty acids. It is one of three primary ketone bodies, the others being acetoacetate and acetone. BHB is a significant energy source for the body, particularly during periods of fasting, prolonged exercise, or when carbohydrate intake is very low, such as in ketogenic diets.

The production of BHB, a process called ketogenesis, increases when glucose availability is limited. The liver converts fatty acids into BHB, which can then be released into the bloodstream and transported to various tissues, including the brain, muscles, and heart. These tissues can then use BHB as an alternative fuel source to glucose. The brain, in particular, can become heavily reliant on BHB for energy when glucose levels are insufficient.

BHB can be converted back into acetyl-CoA within the cells of these tissues, which then enters the citric acid cycle to produce ATP, the energy currency of the cell. The presence of elevated BHB levels in the blood is a hallmark of the metabolic state of ketosis. While BHB is primarily recognized for its role as an energy substrate, research is also exploring its potential signaling functions and therapeutic applications in various physiological and pathological conditions.
