PDE5 inhibition refers to the pharmacological mechanism by which certain drugs work. Phosphodiesterase type 5 (PDE5) is an enzyme found in various tissues, including the smooth muscle cells of blood vessels and the corpus cavernosum of the penis. This enzyme is responsible for breaking down cyclic guanosine monophosphate (cGMP).

cGMP plays a crucial role in vasodilation, meaning it helps blood vessels relax and widen. In the corpus cavernosum, increased cGMP levels lead to smooth muscle relaxation, allowing for increased blood flow and thus, an erection. PDE5 inhibition blocks the action of the PDE5 enzyme, preventing it from breaking down cGMP. This leads to higher and more sustained levels of cGMP, promoting smooth muscle relaxation and vasodilation.

The most well-known application of PDE5 inhibitors is in the treatment of erectile dysfunction (ED). By enhancing the natural erectile response to sexual stimulation, these drugs help men achieve and maintain an erection. Examples of PDE5 inhibitors include sildenafil, tadalafil, vardenafil, and avanafil.

Beyond erectile dysfunction, PDE5 inhibitors are also used to treat pulmonary arterial hypertension (PAH). In PAH, the blood vessels in the lungs are narrowed, leading to high blood pressure in the pulmonary arteries. PDE5 inhibitors can help to relax these vessels, reducing the pressure and improving exercise capacity in patients with PAH. While primarily known for these applications, research continues into other potential uses of PDE5 inhibition.
