Vävnadsfaktorinhibitorn, also known as tissue factor pathway inhibitor (TFPI), is a protein found in the blood that plays a crucial role in regulating the coagulation cascade. It is primarily synthesized by the liver and endothelial cells. TFPI acts as a natural anticoagulant by inhibiting the activity of the tissue factor (TF) and activated factor X (FXa).

The primary mechanism of action for TFPI involves binding to the complex formed by tissue factor and activated factor VII (FVIIa). This binding neutralizes the procoagulant activity of the TF-FVIIa complex, thereby preventing the initiation and propagation of the extrinsic pathway of blood coagulation. TFPI also possesses a separate inhibitory domain that can directly inhibit FXa, further contributing to its anticoagulant effect.

There are several isoforms of TFPI, with TFPI-alpha being the most abundant and biologically active form. TFPI is released into the circulation and can also be found associated with the endothelium. Its regulation is complex and influenced by various factors, including inflammatory signals.

Dysregulation of TFPI activity has been implicated in various thrombotic and bleeding disorders. For instance, reduced TFPI levels or activity can contribute to a hypercoagulable state and increase the risk of thrombosis. Conversely, elevated TFPI levels might be associated with an increased risk of bleeding.
