Rotkanalfyllningar are the fillings placed in a cleaned and shaped root canal after endodontic treatment to seal the root canal system and prevent bacterial leakage. The goal is to create a tight barrier that protects the tooth from reinfection while maintaining canal integrity and supporting the final restoration.

Materials commonly used include gutta-percha as the core filling material and an endodontic sealer. Gutta-percha is a thermoplastic material derived from natural latex, which is inert and biocompatible. Sealers vary and can be epoxy resin-based, zinc oxide eugenol-based, calcium silicate-based (bioceramic), or resin-based; newer bioceramic sealers are valued for biocompatibility and good sealing properties. The combination of gutta-percha and sealer fills the cleaned canal system, including irregularities and lateral canals.

Techniques for obturation include cold lateral condensation, warm vertical compaction, and carrier-based systems. In cold condensation, a master gutta-percha cone is fitted to length and supplemented with accessory cones. In warm techniques, gutta-percha is softened and compacted to adapt to canal anatomy. The canal is dried, the material placed to the working length, and radiographs are used to verify complete and well-sealed fill with minimal voids.

Outcomes depend on obturation quality and coronal seal; inadequate filling or poor coronal restoration can lead to reinfection and retreatment. The practice has evolved with advances in sealers and warming techniques to improve sealing and longevity.
