Osteogenesis is the process by which bone tissue is formed. It is a fundamental biological process essential for skeletal development, growth, and repair. There are two primary modes of osteogenesis: intramembranous ossification and endochondral ossification.

Intramembranous ossification occurs directly from mesenchymal connective tissue. This process is characteristic of the formation of flat bones of the skull, the clavicles, and parts of the mandible. Mesenchymal cells differentiate into osteoblasts, which secrete osteoid, the unmineralized extracellular matrix of bone. This osteoid then mineralizes, trapping osteoblasts within lacunae to become osteocytes. Spicules of bone form and fuse, eventually creating trabecular bone.

Endochondral ossification is a more complex process that involves a hyaline cartilage model. This is the primary mechanism for the formation of long bones, vertebrae, and ribs. Mesenchymal cells first differentiate into chondrocytes, forming a cartilage model of the future bone. This model undergoes growth and development. Subsequently, a primary ossification center forms in the diaphysis, where cartilage is replaced by bone. A secondary ossification center typically develops in the epiphyses later. The epiphyseal plate, a layer of cartilage between the primary and secondary ossification centers, is responsible for longitudinal bone growth. Once growth ceases, the epiphyseal plate ossifies, and the epiphysis fuses with the diaphysis.

Osteogenesis is a continuous process throughout life, essential for bone remodeling and fracture healing. Osteoblasts are the cells responsible for synthesizing new bone matrix, while osteoclasts are responsible for bone resorption. This balance between formation and resorption is critical for maintaining bone health and structural integrity.
