An insulin pen is a handheld device for delivering insulin subcutaneously to people with diabetes. It resembles a pen and holds either a replaceable cartridge or a prefilled insulin cartridge. Dose is prescribed in units, and a dial and button operate the plunger to inject through a replaceable needle. Pens aim to simplify administration and improve dosing accuracy compared with vials and syringes.

There are disposable pens with built-in cartridges and reusable pens that accept replaceable cartridges. They support various insulin formulations, including rapid-acting, short-acting, intermediate-acting, long-acting, and premixed types. Most pens are designed for U-100 insulin, while some models accommodate higher concentrations such as U-200 or U-500.

Using a pen typically involves attaching a new needle, priming to expel air, setting the dose by turning the dial, and injecting at a suitable site. After use, the needle is removed and disposed of safely. Some newer pens offer features such as dose memory, audible confirmation, or optional digital connectivity with apps.

Safety and handling include proper storage, avoiding needle reuse, rotating injection sites, and following disposal regulations. Insulin pens provide portability and reduced injection pain and are used by people with type 1 diabetes and many with type 2 diabetes.
