Acetoasetaattia, also known as acetoacetate, is a ketone body, a type of molecule produced during the breakdown of fats in the liver. It is one of three ketone bodies, the others being beta-hydroxybutyrate and acetone. Acetoacetate is an intermediate in the synthesis and breakdown of fatty acids and is a significant source of energy for the body, particularly during periods of fasting, starvation, or prolonged exercise when glucose availability is limited.

In the body, acetoacetate is synthesized from fatty acids in the mitochondria of liver cells through a process called ketogenesis. It can then be released into the bloodstream and transported to other tissues, such as the brain, muscles, and heart, where it can be converted back into acetyl-CoA and used for energy production via the citric acid cycle. Acetoacetate is also excreted in the urine and breath. Elevated levels of acetoacetate in the blood and urine are characteristic of ketoacidosis, a serious metabolic complication that can occur in uncontrolled diabetes mellitus and other conditions. It is also monitored in medical settings to assess metabolic status and the effectiveness of ketogenic diets.
