Phosphatidylinositol is a phospholipid that forms a key component of cellular membranes. It is a glycerophospholipid composed of a glycerol backbone bearing two fatty acyl chains at the sn-1 and sn-2 positions and a phosphodiester linkage to an inositol head group. The inositol moiety can be phosphorylated at various positions, producing a family of phosphoinositides including PI, PI4P, PI5P, PI(4,5)P2, and PI(3,4,5)P3. These lipids are primarily located on the cytoplasmic face of membranes and function as signaling hubs that recruit proteins with specific lipid-binding domains.

Biosynthesis and metabolism: Phosphatidylinositol is synthesized in the endoplasmic reticulum from CDP-diacylglycerol and myo-inositol by the enzyme phosphatidylinositol synthase. It can be interconverted to phosphorylated derivatives through the actions of lipid kinases and phosphatases. Phosphatidylinositol 4,5-bisphosphate (PI(4,5)P2) is a central substrate for phospholipase C, which cleaves it to generate inositol trisphosphate (IP3) and diacylglycerol (DAG), two important second messengers. PI(4,5)P2 and PI3K-generated PI(3,4,5)P3 also regulate a variety of signaling pathways, including cytoskeletal organization, vesicular trafficking, and cell growth.

Biological roles: As a membrane lipid, phosphatidylinositol and its phosphorylated derivatives orchestrate membrane trafficking, actin dynamics, and signal transduction. They serve as docking sites for proteins with PH, PX, ENTH, and other lipid-binding domains and regulate processes such as endocytosis, exocytosis, and receptor signaling.

Clinical relevance: Alterations in phosphoinositide metabolism are linked to diseases such as cancer, metabolic disorders, and neurodegenerative conditions. Inhibitors targeting phosphoinositide kinases (notably PI3K and related enzymes) are used in therapeutic settings, reflecting the central role of phosphatidylinositol signaling in cellular regulation.
