FKBP12 is a protein of approximately 12 kDa and is widely expressed in various cell types, including skeletal muscle, heart, and liver. The protein consists of a single-domain structure that folds into an eight-stranded β-barrel, which creates a central hydrophobic cavity. This cavity is responsible for the protein's binding capacity to a range of ligands, including the immunosuppressive drug FK506 (tacrolimus).

FKBP12's function as a molecular chaperone is closely associated with its ability to interact with the ryanodine receptor, a calcium channel located in the sarcoplasmic reticulum of muscle cells. Upon activation, FKBP12 binds to the receptor, stabilizing its structure and facilitating calcium release. The interaction between FKBP12 and the ryanodine receptor is also integral to the regulation of smooth muscle contraction.

The protein's two domains, arranged in tandem, are essential for its recognition and binding of FK506. The sequestration of FKBP12 by the immunosuppressive drug creates a complex that interferes with the calcineurin activation/initiation component of the T cell activation pathway, disabling the proliferation of T-lymphocytes.
