Factor concentrates are medical products consisting of highly concentrated coagulation factors used to prevent or treat bleeding in people with clotting factor deficiencies. They are most commonly used for hemophilia A (factor VIII deficiency) and hemophilia B (factor IX deficiency), but can also treat other rare factor deficiencies or for surgical prophylaxis.

There are two main production methods. Plasma-derived concentrates are obtained from donated human plasma and undergo viral inactivation and purification steps to improve safety. Recombinant concentrates are produced using genetic engineering in cell lines, avoiding direct use of human blood. Concentrates may be single-factor (e.g., FVIII or FIX) or multi-factor preparations, such as prothrombin complex concentrates that supply several coagulation factors, or activated concentrates used in specific clinical situations.

Clinical use includes routine prophylaxis to prevent bleeding, on-demand treatment of spontaneous or trauma-related bleeds, and perioperative management. Dosing and scheduling depend on the patient’s factor level targets, weight, and the severity of deficiency. Administration is by intravenous infusion, typically in a hospital or home setting with trained personnel.

Safety and regulation are governed by national authorities, with extensive testing for viral safety and potency. Potential risks include allergic reactions, infectious risk (historically), and the development of inhibitors (alloantibodies) to infused factors, which can reduce treatment effectiveness. Storage conditions and shelf life vary by product.

Factor concentrates have substantially improved outcomes for people with bleeding disorders, enabling long-term prophylaxis and improved quality of life, while requiring careful monitoring and individualized treatment plans.
